چکیده

A case of anti-basement membrane disease with severe pulmonary haemorrhage and normal renal function is reported. In 1919 Goodpasture described a syndrome of glomerulonephritis and pulmonary haemorrhage.' Half a century later both manifestations were shown to be induced by circulating autoantibodies that reacted with alveolar and glomerular basement membranes.2 We present a case with severe pulmonary haemorrhage mediated by anti-basement membrane antibody in which renal function was normal. Case report A previously healthy 20 year old cigarette smoking farmer was referred complaining of dyspnoea and progressive haemoptysis of four days' duration after accidental exposure to fumes of fungicides containing metalaxyl (8%) and mancozeb (64%). He was pale, weak, sweating, and febrile (38 5°C). His pulse rate was 104/min, blood pressure 110/50 mm Hg, and respiratory rate 26/min. A chest radiograph showed diffuse fluffy infiltrates predominantly in the lower lobes. The erythrocyte sedimentation rate was 80 mm in one 68 group.bmj.com on June 25, 2017 Published by http://thorax.bmj.com/ Downloaded from

Venous thromboembolism with concurrent pulmonary haemorrhage in systemic vasculitis.

BACKGROUND Pulmonary haemorrhage (PH) is a serious manifestation of systemic vasculitis with high mortality rates yet vasculitis is associated with an increased prevalence of venous thromboembolism (VTE). The concurrent presentation of severe PH and VTE poses a challenge in terms of therapeutic management. METHODS This is a retrospective case review of the clinical manifestations and response...

CASE REPORT Alveolar haemorrhage in anti-glomerular basement membrane disease without detectable antibodies by conventional assays

Anti-glomerular basement membrane (anti-GBM) disease represents the spectrum of disease attributable to circulating anti-GBM antibodies. While active anti-GBM disease in the absence of circulating anti-GBM antibodies has been described, it is considered rare with the use of current routinely available assays. We report four subjects with features consistent with active anti-GBM antibody disease...

Anti-GBM-associated crescentic glomerulonephritis with discrete IgG deposition, but with no electron-dense material in glomeruli.

Crescentic glomerulonephritis is characterized by extracapillary proliferation resulting from severe inflammatory injury of various aetiologies. It is usually rapidly progressive. Meanwhile, anti-glomerular basement membrane (GBM) antibody-mediated glomerular lesions induce crescentic glomerulonephritis. When associated with pulmonary haemorrhage, it is given the eponym 'Goodpasture's syndrome'...

Recurrence of anti-GBM disease 8 years after renal transplantation.

Anti-glomerular basement membrane (GBM) disease is a disorder characterized by antibodies against an epitope of type IV collagen found on the GBM. The major clinical sequela is rapidly progressive glomerulonephritis, which may be accompanied by pulmonary haemorrhage (Goodpasture’s syndrome). Glomerulonephritis secondary to anti-GBM disease frequently progresses to end-stage renal disease (ESRD)...
